Grant Contract Proposal Cover 

Applications submitted to the NDOT-HSO must include a completed Grant Contract Proposal Cover.

 

 

Grant Funded Enforcement Activity Summary

Grant funded enforcement activity must be reported on this form.

 

 

Grant Funded Education Activity Summary

Grant funded education must be reported on this form.

 

 

Grant Funded Training Conference Evaluation Report Form

This report must accompany all claims for travel reimbursement and must be attached to the Claim for Reimbursement/Mini-Grant Claim for Reimbursement.

 

 

Project Grant Forms

 

Annual Report Form  

An annual report must contain all accomplishments and activities utilized to meet the target/objectives outlined in the Grant Contract Proposal. Each objective must be addressed and the project's measurable impact on traffic safety must be included. All grant contractors must submit an annual report to the NDOT-HSO no later than the second Thursday in November of the following fiscal year.

 

 

Personal Services Breakdown  

Reimbursement requests for personal services must utilize this form.

 

 

 

Weekly Time Report  

Reimbursement requests for personal services must be substantiated by submitting a completed Weekly Time Report form with appropriate signatures.

 

Grant Contract Claim for Reimbursement  

The grant contractor is responsible for submitting the Grant Contract Claim for Reimbursement (CR) monthly with a copy of supporting documentation substantiating expenditures associated with the project. The CR request must be completed and submitted to the NDOT-HSO on a monthly basis. CRs are due by the 10th of each month. CRs received after the 10th of each month may not be reimbursed until the following month.

 

 

Equipment Inventory Log 

All equipment purchases must be specifically itemized in the budget proposal. If not included in the original budget proposal, expressed written approval from the NDOT-HSO is required prior to any purchase. Competitive bids are required on any equipment purchase in excess of $500.00. Upon receipt of any equipment, an "Equipment Inventory Log" will be completed by the grant contractor and submitted to the NDOT-HSO. The grant contractor shall appropriately maintain any equipment purchased under the grant contract. Mini-Grant Forms

 

Mini-Grant Contract Claim for Reimbursement (CR)  

The mini-grant contractor is responsible for submitting the Mini-Grant Claim for Reimbursement (CR) with a copy of supporting documentation substantiating expenditures associated with each mini-grant contract issues to them. The request(s) must be completed and submitted to the NDOT-HSO as reimbursement is needed and/or upon completion of the mini-grant contract. CRs are processed upon receipt of the correct documentation.
